The MYC Membership Application form is shown below, for your information.
We encourage you to download the PDF document (to the right), which is a form that can be filled out electronically and submitted by email.
If electronic processing is not an option, you may download and print the PDF document, fill it out by hand, and mail it in.

This is not a web form, ie you do not fill it out and submit it using a web browser.
Please download the PDF application document using the Download button at the top of the page.
Open the PDF document in Abobe Reader (or another PDF app), and fill in the fields electronically.
Then save the filled-in document, and email it as an email attachment, following the instructions on the application.
Alternatively, you can download the application, print the PDF document, fill it out by hand, and mail it to the MYC PO box (as was done in the past).
